Could be driver related, what driver are you running?

Could be your card has lost it's VGA setting? have you shut the pc down incorrectly recently (just by pressing the case power button).

Could be your monitor settings, are you running at 60hz?

If you answer the questions posed in my first post I'll be better placed to advise.

Yes update drivers.
Also check to see if the heatsink and fan on the graphics card is free form dust.
It is possible that overheating is causing the problem, thats why it happens during games(the game puts the graphics card under load and the card heats up).
Check all fans and heatsinks for dust while you are at it.

To check driver version go to Control Panel/System/Hardware/Device Manager/Display adapters/right click properties/click Driver.

To check refresh rate go to Control Panel/Display/Settings/Advanced/Monitor.
