It must be something wrong with the USB ports, Are they ports connectected directly to the motherboard, or is it an extra USB card?

Sounds like the usb ports have decided to run away and join the circus, you have 2 choices if they really are had it, either replace the motherboard, or the cheaper and easier option is to buy a pci usb card and plug that into your motherboard

In the device manager you will see that the usb ports will be installed and if you look at the properties it will say they are working correctly ect but what you may find that is the problem is that the usb ports contacts are worn, this is caused through the plugging in and unplugging of devices. I have had this problem myself, if your not very confident at taking your computer apart and using a precision screw driver to manipulate the terminals then i would go with the adding a pci usb card

You need to take the motherboard out of the pc and make sure you have some good light, check the connector to the rear of the usb ports are making contact with the motherboard, if they are not you will need to re-soldier them (easier said than done there) if this is all in order the next check is inside the usb port itself, checking the contact are all there and that they have not been pushed to far back so they don't make contact, you can use a small presision screwdriver to gently lever them out slightly,
